FHI 360’s National Institute for Work and Learning (NIWL) coordinates Registered Apprenticeship programs with partners – including the U.S. Department of Labor, National Youth Employment Coalition, National Association of Workforce Boards, National Association of Workforce Development Professionals, National Coalition for Transformative Youth Policy, New Ways to Work, Public Works Alliance, and additional affiliated partners – as part of the Apprenticeship Build America 2 (ABA2) grant, awarded by the U.S. Department of Labor.
By this Communications Release, I hereby grant permission for NIWL’s Registered Apprenticeship programs and affiliated partners to record my 1) name; 2) photographic, video, or audio image, picture, and likeness (the “Images”); and 3) quotes, text, and story (the “Story”). I further grant NIWL’s Registered Apprenticeship programs and partners permission to use and distribute my name, Images, and Story in any communications pertaining to the ABA2 grant, including external or internal promotion, advertising, or marketing, any of the partners’ websites, and training materials. I understand that this potentially includes websites, social media, television, radio, print materials, and events.
I agree that the Images and Story, and all copyrights embodied therein, will be the property of FHI 360 and hereby release any rights that I may have with respect to the Images. If I obtain a copy of the Images or Story, I shall not use them or authorize their use by anyone else.
I understand that FHI 360 is located in the United States and therefore my data will be stored on servers located in the United States. FHI 360, the U.S. Department of Labor, and affiliated partners will have access to this information in a way that is identifiable.
I acknowledge that my participation in recording the Images and Story is purely voluntary and that I will not receive any compensation for my participation. I waive any opportunity to inspect or approve NIWL’s Registered Apprenticeship programs use of the Images or Story. I agree to release FHI 360, their affiliates, employees, and agents from any liability, loss or damage caused by or arising from use of the Images by NIWL’s Registered Apprenticeship programs.
